Composite activity index from a 7-day symptom diary.
Symptomatic disease activity in adult Crohn's disease, over the preceding seven days. Historically the primary endpoint in Crohn's trials; in practice it is used mainly where a standardised, comparable activity measure is needed.

Any item left blank stops the score being calculated — an incomplete form never reports as remission.
| < 150 | Clinical Remission |
| 150 - 219 | Mildly Active |
| 220 - 450 | Moderately Active |
| > 450 | Severely Active |
Bands as implemented in IBD Care. A band is a description of the score, not an instruction.
Requires a seven-day patient diary, so it cannot be completed at a single visit. It correlates poorly with endoscopic activity: a patient can sit below 150 with ulcerated mucosa, and the general-wellbeing and abdominal-pain items carry enough weight that coexisting irritable bowel symptoms inflate the score. Not validated for isolated perianal disease.
