Full Mayo including the endoscopic subscore (0-3).
UCEndoscopicValidated score
Clinical indication
Disease activity in ulcerative colitis including the endoscopic subscore, giving a combined clinical and endoscopic measure. Widely used as a trial endpoint.
What is scored
The calculator collects 4 items:
Stool Frequency
Rectal Bleeding
Endoscopy Findings
Physician's Global Assessment
Any item left blank stops the score being calculated — an incomplete form never reports as remission.
Interpretation
≤ 2, no subscore > 1
Remission
3 - 5
Mild
6 - 10
Moderate
11 - 12
Severe
Bands as implemented in IBD Care. A band is a description of the score, not an instruction.
Limitations
The endoscopic subscore carries well-documented interobserver variability, particularly between 0 and 1, which matters because that boundary now defines endoscopic remission in most trials. Friability placement differs between the original and later interpretations. The physician global assessment is subjective.

The Mayo score was described within this mesalamine trial rather than in a dedicated methods paper, which is why the title does not name it.
