PDAI without histology; 5 or more diagnoses pouchitis.
UCPouchValidated score
Clinical indication
Diagnosis of pouchitis without the histological subscore, for settings where histology is not immediately available.
What is scored
The calculator collects 10 items:
Stool frequency compared with the usual post-operative pattern
Rectal bleeding
Faecal urgency or abdominal cramps
Fever
Pouchoscopy - oedema
Pouchoscopy - granularity
Pouchoscopy - friability
Pouchoscopy - loss of vascular pattern
Pouchoscopy - mucous exudate
Pouchoscopy - ulceration
Any item left blank stops the score being calculated — an incomplete form never reports as remission.
Interpretation
< 5
No pouchitis
≥ 5
Pouchitis
Bands as implemented in IBD Care. A band is a description of the score, not an instruction.
Limitations
Dropping histology costs some specificity against the full PDAI. Like the PDAI it identifies inflammation but not its cause, and the same differential — cuffitis, Crohn's of the pouch, irritable pouch syndrome — still has to be worked through separately.
